I know it's well established that the 2.7s run rough after a battery replacement. ECU relearning and whatnot.

The problem i have, is on my truck it takes a week or more of normal daily driving to go back to normal. And it seems to be getting worse as the truck gets older. I had my battery out on Monday, and it's Thursday and she still idles (very shakily) at around 300 rpm, occasionally stalls out at stoplights, and is difficult to start.

I know it will eventually return to normal. It always does. But in the meantime, it's barely drivable for what seems like way too long.

    Anybody else have this issue?

Clean the throttle body, a dirty throttle body is the most likely cause.

I second the throttle body cleaning - maybe also clean your MAF sensor with DESIGNATED MAF cleaner... not just brake clean. If issue persists, disconnect the battery and zip tie the positive and negative cables together (make sure they're touching each other) for 15 minutes... all those combined should help significantly.

So far…. Added fog lights swapped halogens to led’s, interior lights to led’s, bed lights added, led/strobe brake lights, trying to figure out the right leds for turn signals.
May want to get the Device that plugs into your cig lighter and keeps the computer settings alive w a 9volt battery or something like that. Not sure if it would help or not?

It would have to be one that plugs into the OBD port, 12v outlets aren't powered on a Tacoma with the key off.
If the TB is that dirty that it runs poorly after a battery disconnect it should be cleaned anyway, on a vehicle with a clean TB and no underlying issues that the PCM has compensated for clearing the Keep Alive Memory by disconnecting the battery should cause no driveability problems.

I haven't cleaned the TB because it finally shaped up shortly after posting this haha.

I have however cleaned the maf in the last year.

Truck has 252k and the TB has never been cleaned. My gas mileage has been a touch low of late so it's definitely on the list.
