That's typical for this motor from the factory. They look like they've got a few miles on'm... Any idea how many? Manual says 30k.

Like @Larzzzz said, normal from factory. This is the first time you've changed them?

Since you have that nifty scanner, take your diagnostics a step further and check short term and long term fuel trims. Ensure your vaccuum system/ intake is sealed and leak free. If it is and your short terms and long terms are in the 30's-40's+ you want them within a +-5% range, 0 is desired these days by most all manufacturers, check fuel pressure. You want to see about 41 psi. If that looks good consider changing the MAF out.

Problem is, at 165k, it could either be the fuel pump not allowing enough volume or the MAF detecting a lack of air. Both have been known to give issues and fail in the past. Simply cleaning the MAF may not take care of it as they can eventually lose calibration in time.

Just did a 30k service. From the looks of it you already had the new plugs. But the 4.0 seems to like these....

Did I miss where you checked for vacuum leaks, around the engine?

I already know that a lot of your vacuum hoses have split ends.
