Are those throttle by wire? Does it run good, just sluggish?

I’m triying to figure it out since car is out of warranty with Toyota , and inspection runs around 150+ , could use that to fix the car instead

Does it shake while accelerating? They have issues with torque conveters.

I’ve noticed rough idle under load, specially in reverse , once in neutral or park it goes away

Check all the vacuum lines. My 2011 ran rough and even shut off, turns out one of the silencers plumbed along the intake popped off and threw everything for a loop.

It is kinda weird that you have no CEL. Along with the low miles. Somewhat challenging. My daughter's RAV4 had problems with a bad spark plug but it had 140k on it and was definitely lighting up the dash. If I had your problem I would do the basics. Fuel filter, check fuel pressure, replace plugs.

Quick way to diag cats is to remove the upstream oxygen sensor, and rev it.

Clean the MAF sensor and throttle body butterfly valve. Build a smoke tester to check for vacuum leaks if you want. If you have a scan tool with real time data, check throttle position and intake air flow rate at idle and at full throttle. If nothing abnormal is found, take it to a shop. Don't throw parts at it.
