About the only thing to look at is fuel trims.
That would be one way to tell if there is an issue.

I get about 11 in stop and go city. Almost 20 highway at 65mph trips.

If you do only city driving in a lot of stop and go traffic 11 mpg isn't far off what I'd expect. I never drive in those conditions but will get 16ish in a mix of small town and rural 2 lane roads. Open road driving is far different. 19-20 is more likely.

FWIW even the cars advertised to get 40 MPG won't get 20 mpg in those conditions.

Where do you live and what is the fuel law? Affected states should be transitioned to winter-blend fuel now. And then if you pumped winter-blend E15, 12-13mpg is about the best combined you'll see.

What is on and in your truck, how heavy is your foot, and what wheels tires and air pressure do you have?

Is your truck stock? Re geared? Big tires? Lifted?
