I have a 2013 TRD and I was getting misfire codes on Cyl 1 & 6. I had the plugs, coil overs changed, change O2 sensor bank 1 sensor 1 and had it flushed. When I got my truck back from the mechanic I was getting just a P2238 code intermittently. Took it back to the mechanic and they replaced the O2 sensor for a second time. Problem still persisted. When I took it back a third time the replaced both sensors on bank one. They said that if that didn't fix the problem to take it to Toyota. The problem is not fixed now the instead of the check engine and track off lights coming in intermittently they are on permanently and there is a second code P2195. So there are now the P2238 and the P2195 codes on permanently. Does anyone have any suggestions. I'm trying to avoid taking the truck to Toyota to get looked at. Any advice would be much appreciated. I'm somewhat mechanically inclined I just didn't have the time to fix the other issues. I need to register and get a smog by the end of the month. Please help!

    Welcome to Tacoma World.

According to the thread below, someone else fixed P2195 by having Toyota replace the A/F sensor. May be something to look into.

P2238 could be a number of things but seems to be either a weak battery, temporary issue, bad connection at the plug, or something else that we wouldn't know over the internet. May take some research and trial/error on your end.

Did the mechanic who replaced the O2 sensors use Denso OEM sensors or something else? Seems like they can be touchy and prefer the Denso parts.

This, if the code wasn't present prior to changing the sensor I'd make sure the correct sensor was installed.

Thank you both very much for the info. I took the Tacoma to another mechanic. They have had it for 4 days. The track off and check engine light is still on. And they can find nothing wrong with the truck. So that's two different mechanic shops. And I still have no idea why the track off and check engine light are on. There is a permanent p2238 code that still is popping up. All of the sensors that were put on which were two. Bank one sensor one has been replaced three times. Bank one sensor two once. And they are both denzo parts.

I also asked this last mechanic. If it could be the ECM. And he said that seems to be functioning properly also. They literally could find nothing wrong with the vehicle. But it's still throwing a permanent code p2238. Track off and check engine light are on.

You may want to check the circuit integrity to the (Bank 1) A/F Sensor.
You may have a shorted or open circuit.

Thank you I will check the integrity of that if that sensor. And I will report back. Could it be something with my fuel injectors? Or a small exhaust leak. I have a p2195 stuck lean code and the 2238 codes that are both permanent. I just cleaned my mass air flow sensor and it was dirty. But the check engine and track off light came back on fairly quickly after I cleared the codes. I'm at a loss because everything seems to be getting the proper currents to the bank one sensor one sensor. I'm confused as hell and so are three different shops at this point. Thanks for any feedback back at all. I appreciate it tremendously!
