Hate to be added to the list but it sounds like I might have to be. I feel so validated after seeing all these posts with the same problem, seems like its no surprise to Toyota despite them telling me they have never seen it. Here is a link to my post outlining what happened and where I'm at if anyone can provide some advice and guidance, I've added the mechanic notes they sent me.
    https://www.tacomaworld.com/threads/random-engine-misfire-on-a-2018-trd-sport.825957/
    Here's the long story short

2018 TRD sport Tacoma with 77k miles on it. Has been babied, no towing no off-roading just a daily driver stock. Was driving back on a road trip and about 100 miles from home the engine light flashed while we were going 65 on the highway. Was driving super smooth beforehand. Had to get it towed to the dealership near me and its still there here is where its at.

    -checked and replaced spark plugs
-swapped coils and still got a misfire
-said they checked all external parts for leaks
-still had a misfire and used bore scope and saw scoring in cylinder 2 so sure enough needs a new engine

I was shocked, babied the truck, oil change every 5k on the nose.

Here's the kicker that would be really helpful if anyone can reply. I bought it CPO at 50k, so it has the 7 year 100k powertrain warranty. The dealership called Toyota Warranty to see if its covered and Toyota got back with they need to find the exact point of failure and do a teardown that can take up to 14 hours of labor. If they find it and send it to Toyota then it may be covered (though the powertrain would lead me to think it would be). Okay so Russian Roulette with 3k bill of labor and maybe coverage is leaving me super uneasy. Also, they thought initially it was spark plugs so they replaced them and I have a spark plus, labor, and 2 hours of diagnosing the engine failure after that, that could esaily be 1.5---2k (I'm gonna fight for coverage but idk) It looks like thanks to this forum it is looking like I am in the boat of issues listed that should be covered. But if anyone can also let me know the treatment they got and what was covered to know what I should expect. I called Toyota Corporate and said I was incredibly disappointed with the potential 3k labor change and they are calling the dealership to see what they can do.

    Thanks all

Here's something interesting a Toyota master tech and me had talked about while discussing what had happened to my engine.

Apparently 2016 and 2017 are known for this. It's the one big issue with Tacoma's that he knew of.

"If it happens within the warranty period Toyota replaced the heads. The machined bore for the hydraulic lash adjusters for the cam followers (rockers) can cause the adjuster to stick, causing the rocker to become loose, and possibly come apart. For some reason it isn't a well known issue but I've seen it many times. You will hear a LOUD ticking, almost knocking beforehand. I've worked on 3 of them that were about to fall apart and made a ticking sound, and Toyota would not go good for it until they fell apart."

Interestingly enough, he nailed what happened with my engine right before it failed to a tee.

It made a loud ticking noise that was almost a hard hammering tick, but I was told was normal and was the high pressure fuel pump.

Thanks for the info man, I certainly do hear a LOUD knocking sound and engine chugging. I got no light indicating the engine temp was in the red or anything. Very sudden situation where check engine started flashing and ABS trac light came on. The lead Toyota tech working on the engine will do a tear down on Monday so I will keep you all updated.

After startup CEL on, check codes missfire and generic code. Called toyota and reported the issue. Deleted codes to replicate, could not replicate it. Dropped off truck the following day. They provided a Highlander as a loaner.

Next day service advisor calls and updates me that Toyota regional is dispatching an FE to troubleshoot.

Following day service advisor calls for another update - FE found abnormal wears in the cylinder head caused by the valves. They replaced my whole cylinder head.

I had an existing appointment at a later date for my leaking transfer case.

Total time the truck was in the shop was about two weeks. All covered under WA.

Light towing, towed something atleast 6 times.
Light offroad. Truck was never abused.

    All stock.

My engine didn't totally fail but I think I should be added to the list. I had a 130psi compression leak down result in cylinder 3 with a code P11EE CEL first noticed around 41,000 miles. CEL initially appeared a few days after I filled up with 92 octane and made a couple hard pulls onto the highway. Pretty normal driver otherwise. 2018. Still drives fine and have just reset the CEL about once every tank of gas or about every 250 miles. Story here: https://www.tacomaworld.com/threads/my-p11ee-cel-at-41k-miles-on-2018-or.829351/

Cylinder #3 misfire, high rpm under load..

While performing troubleshooting, the symptom was strange. The misfires were changing cylinders as I swapped coil packs around, making me believe the coils were just going bad.. and when I pulled a coil, there were arcing marks on the bottom of the sleeve where it meets the boot.. but the only cylinder that coded was Cyl. #3
So glad I was able to find a similar issue on this forum!

ACIS (Acoustic Control Induction System) failure

If only the failure points were reinforced or made of metal in later model years but I don't think they are. Looking forward to this issue in 4 to 5 years! :kona: Could be worse, at least I don't have active grill shutters I suppose.

Add me to the list... I had the Issue with ACIS failure.
2017 Sport. Lifted ( 3inch) 33's no other real modification that would matter here.
    82k miles
    highway use

Started last year in the fall truck went into limp mode and had codes for mis-fire on cylinder 3. Had it towed to the shop and they ( for some reason) only replaced the coil on that one cylinder. Then in February on my way to a camping trip same shit... this time I made them replace all the coils. Then again in March same issue... this time they found this issue and replaced the entire part but said they couldn't find the flap. Fathers day weekend camping again ( maybe I should give up camping, at least with this pos truck) I broke down.. honestly it sounded like a belt broke or I threw a rod. Its back at the shop for the last 3 weeks and they can't figure out the issue other than its throwing mis-fire codes still.

    2023 Tacoma Off Road
    49k miles all highway miles
Bone stock, regular maintenance up to date

P11EC CEL came on along with a misfire, dropped it off at the dealership this morning, valve got loose and dropped, somehow truck was still running, they’re going to replace the head under powertrain warranty

That is crazy. You can't just replace a head if a valve was riding on a piston.

Good luck and ask for pics.

I've been dealing with ongoing engine issues with my 2018 v6 Toyota Tacoma (2GR-FKS engine) The first major issue occurred around 88k miles with a strange noise from the engine, leading to a timing chain & tensioner replacement at my own expense. Despite multiple repairs, including 2 cam phasers , 2 vacuum pumps,(all covered under warranty) and hydraulic lifters replaced (partially covered under warranty), the issues persisted. At 150k miles now, the vehicle is now idling rough, and I’ve been told Cylinder 6 is pitted and scoured, likely due to piston ring failure. The valve covers constantly leaking oil with with knocking sounds, even after multiple attempts at repair. I’m frustrated with the repeated failures and the mounting costs for repairs. Add me to the list.
