I have a 2023 Tacoma 4x4...the dealer wants to do a 30,000 mile service for the 4 wheel drive system...is that a real thing?

    Yes.

    It’s not a difficult DIY job if you’re able/interested.

Fluids, front diff, rear diff, and transfer case. DIY, not hard, just do your research, the T-case needs special straight 75w fluid. Ski

Stock wheels and suspension. Random "utilitarian" mods featured on this great forum.
It is... Your truck has two differentials - front and rear along with a transfer case. Many including myself have done a 30k mi (or sooner) diff fluid change. Some also do the transfer case but that fluid comes out pretty pristine so waiting till 50k mi if you don't do heavy towing should be ok.

Pretty easy job overall. Front drain can be a bit of a pain to brake loose.

Bingo. I did mine over 3 different days (including my manual trans fluid). Each step took an hour or two while taking my time. The fluids are expensive, and so is a dealership's labor rate.

Did this recently .. transfer case and rear are pretty easy . The front can be a bit challenging since it’s tucked away . Fluids u can do 75w 90 for the front and rear diff . The transfer case u can use the redline mt-lv way cheaper than the oem gold fluid which is like 90-110 a quart . Who knows why it’s that expensive .depending on the dealer price will be about the same after purchasing the fluids and tools needed to complete the job . I wanted more of a piece of mind and use the brand fluid I wanted over what dealer was offering I did it myself .

All the dealer will do is inspect the fluid in your t case. If you don’t ride your rig hard, they won’t change your fluid. Earlier 3rd gen models, they will also inspect drive shaft and lube it if has grease zirks.

One piece of advice I keep in mind for when I self-service ... FIRST verify you can remove the "fill" port BEFORE opening the "drain" port. There is no guarantee that the "fill" port opens at all.
