My '06 has 155k on it. A few months ago started having intermittent trouble with the blower. I pulled the interior air filter and it was trash. I had a little debris below the air filter ( mainly a few leaves) I removed those and spun the fan with my hand. I turned switch on and it worked fine and has for 2 months. Now it has quit again. But if I remove filter and spin blower by hand it works on all speeds until your turn ignition switch off. When you turn it back on nothing. Spin blower by hand, turn switch on and it works fine, again on all speeds. I might add that the lights that illuminates the switch is burned out and has been for years, I just never went through trouble pull dash apart to replace. So do I have a bad switch, weak motor, something else. Thanks for any info.

When you spin it by hand it loosens up; ignition on and it runs on all speeds so the resistor is ok; then while you're running it it heats up the bushings (don't think it has bearings) and then you shut it down; friction seizes the old bushings; won't start without a push. (also could have an open winding-dead spot)
    Motor is shot.

Agreed, if you spin it and it runs, especially on all speeds, it's probably the motor. Just for giggles, wiggle your resistor plug some time when it won't start and see if it starts. If it does, look at the pins inside of the connector for a burn streak and loose pins.
