Start the hole with drill in reverse. Once you have a good start, drill normally but SLOW. Do not force it. Done

Can I score the fender with the saw in reverse, and then change direction?

Exactly. And ffs, if you have never used a hole saw before, at least practice on a piece of wood before trying to drill your fender.

My opinion: Too much work using a Dremel for this particular fender project. I would've used a angle grinder with a fresh cutting wheel since there is a lot of room and a lot of relatively straight lines. For the inside curves, maybe switch to a grinder, instead of cutting wheel.

For a 4" hole, the Dremel method would likely work a lot better than a angle grinder because there's not a lot of room for it.

    Just an opinion.

Just to be clear for everyone who isn't familiar with this method. You need to have the drill in forward in order to make the center hole with the arbor. Once the arbor is through, then you reverse to score your hole saw line, then forward again to complete the cut.

If you attempt to drill the arbor (pilot) hole in reverse it's just gonna walk all over the place on you. And start with very little pressure until the arbor bit starts to cut where you want it too. If you press too hard and it starts to walk it may slip and scratch the hell out of your truck.

When I have to drill ceramic tile for mounting grab bars in showers I use small diamond holesaws to get through the tile. They don't have arbors. You have to have the drill going full speed and contact the tile at a 45° angle until you get a score line, then straighten out slowly to go straight through. Stresses me out every time I have to do it. Especially in a brand new remodeled shower.

Good stuff. Additional words of wisdom if you don't mind, mainly to reduce the chances of wandering:
Use a center punch to make a small dimple so your pilot bit doesn't wander around.
After you center punch, maybe drill a hole with a regular, sharp bit.
Then follow with your hole saw, going in reverse as previously described, with the pilot bit into your new center hole.
Then forward to complete the cut as described above.

Yep. I like doing an 1/8" pilot hole as a first step any time I'm drilling metal. Easier to control and it removes the possibility of the larger bit walking around on you.
