I’m going to replace my rack and pinion soon (small leak) and was considering replacing the pump too. The pump seems fine, but it does have nearly 200k on it and it seems like it would buy me some assurance for the new rack/pinion. Has anyone here actually had their pump wear out though? That’s an extra $300 I could put elsewhere if these things last forever.

I'd just replace it. Not so much to protect the rack/pinion but because it's 15 years old.

I'd 100% leave it. It's probably happened, but I've never heard of a PS pump "wearing out" as long as it had fluid in it. My last DD had 313K on the OE PS pump and was working fine when I sold the car. I did replace the O-rings at some point as it was leaking, but it looked new inside since everything is essentially submerged in fluid.
