heard a loud metal sound coming from front suspension, after inspecting found that the nut on one of the struts seemed to be treaded all the way but the washer was loose and moves up and down freely. Any ideas of what might have cause this and how to fix it?

you talking about the 3 nuts on the hat top or the center one? tighten them back down and keep going.
Suspension bolts can loosen up, you are supposed to inspect them 100 or so miles after install and at an interval that works for you, check them every oil change for instance.

check the cir-clip at the lower end of the shock and the perch where the base of the coil sits. It should sit flat and even in one of those grooves.
Yeah, the center bolt is usually under a lot of tension, was this a recent install?

Yes it was recently installed, I noticed today they forgot to add the lower washer on the strut.
