    If that bolt was loose it could explain alot.
    If there was work done on the front probably dosnt recheck torque after a few miles.

    If he didnt torque the bolts and check them again it makes total since.

    tonneau cover.
    Something not adding up. I've had off road damage repaired under warranty even when I expected to pay. I was told that based on Toyota advertising the use of the truck in such situations they are obligated to repair damage caused while using it under those same conditions. IF the truck is unaltered.

    Water is one of the heaviest elements and when you try to use the truck as a snowplow...its gonna break. Have to agree with Toyota on this one since you gave them the evidence to deny the claim.

    Working on them
    I have had Differential Drops on a couple Toyota's (Fj Cruiser and Tacoma 1st gen) that I have driven hard off road so when properly installed I kinda am hard pressed to fall for that they are the Cause of breakage alone. I always been told two two things about when stuck with a Big First is do not accelerate forward then sudden reverse and back to forward <---- auto trans folks love to do that reason is because that can apply a tremendous amount of torque on things = Snap! and of course don't spin the hell out of things and let it hit dead traction for the same reason. So what is missing in the story? :der:
