Oops, it's the 6 lugger 4 cyls where the auto is rated higher. Nevermind.

Wonder how I never saw that 3.58 spec before. I must not be looking at automatics much.

Nope. Just look at the X-Runner: lower-profile car tires that are lighter, with less rolling resistance; the whole truck is low to the ground to reduce drag. Yet the gas milage is identical to a V6 PreRunner.

IMO the weight of the tire/wheel is a major factor. When I bought my wheels/tires I did my research trying to maintain my mpg's as best as possible with the upgrade.

To the best of my knowledge my wheel/tire combo was only about 4 lb's heavier a piece. My mileage dropped about 1/2 mpg. I looked at several tires and it amazed me at the difference in the weight of each. Needless to say my Geolanders were the lightest I could find that I liked.

My guess is the x-runner and pre-runner aren't separate EPA tests. X-runner gets the same rating by default.

Just like an access cab pre-runner I4 stick got the same EPA as a reg cab 5-lug I4 stick.

^I think you are correct. The EPA distinguishes vehicles by weight class, transmission and basic engine. This means X-runner and PreRunner are treated the same. Access cab and double cab are treated the same (GVWR are the same). But 4x4 and 4x2 need to be listed differently. In theory, this means if Toyota made a special trim level with factory supercharger (like the TRD Hilux), they may not need to test it for fuel economy, because the "basic engine" is the same.
