I have the key inserted in the pic. The tumbler? (please correct me if I’m using the wrong term) that is in the middle is still raised. It should be flat or level with the others, correct?
If so, is there anything I can do to fix it?

Not sure what happened or rather, why it worked. The key slot was not straight, so I inserted the tip of the key, straightened it & now it's working fine.

Glad you figured it out. I have 8 of these door lock cylinders in front of me =)

I know you said you got it working again, but I would suspect that your key is worn a little and the lock itself may be fine. If that ends up being true, you can take your key to a good locksmith and they should be able to tell what the cuts are supposed to be without having to recut keys from your VIN, or another source. I re-did all my locks to match one key back when I first bought my Tacoma.
