Should I be concerned about this rust on the frame of my 2001 Tacoma or is it just surface rust?

At a minimum I would hose that baby down with fluid film. What else you do depends on the condition of the rest of the frame and how long you want the truck to last.

Clean it up and get Waxoyl for inside and outside frame. Doesn't look bad what you showed.
