OP, tell them you want to be there when they start it up. Yes, you’ll have to leave it there over night, but this gives you the best offense. You can be there in that moment it occurs, and they can’t tell you that they can’t replicate it.

And remember, you’re not married to that dealership. It is hard to find a gem dealership/stout techs, but their out there.

Yeah, I've had a few things denied before that were considered wear items even though there was no way they should have worn that quickly (trim pieces). But I came out ahead here and that was refreshing. All they had to do was make a few clicks on the computer and Toyota said they'd cover it. Even better considering I didn't even buy the warranty through that dealership.

Yeah, although it still shouldn't be happening. Best as I can tell, the leaking part is some sort of plastic fitting that wouldn't have a clamp or anything either. Hard to see for certain though with where it's at.

Appointment is Friday. Near as I can tell, it is coming from one of the two black plastic fittings on the back side of the block. No idea what they are. I also found a bracket that is missing a bolt (likely unrelated) and I have the bolt I believe goes to it. But I also can't reach that so hopefully that'll be addressed too.

Dropped it off today and showed them a video of what I could see happening. This time they let me speak with the actual tech that is working on it. He thinks he knows what's wrong with it and believes it is possibly a cracked or loose hard line (high pressure line, possibly as suggested earlier in the thread). However, he can't replicate the issue at the moment because the truck is too warm so he is going to wait until the truck cools off (maybe Monday) and try to confirm his suspicion. In the meantime I have a loaner and everything is still covered, by the sounds of it. They also said they would try and further diagnose the belt sound.

I also failed to mention earlier that I had found a bolt dropped out from a bracket and stuck between the car and engine bay. He confirmed that the bracket holds the wiring harness and wasn't super important, but they would try to get it fixed as well.

With that much flame you will need to grill or cook Santa Maria style. Too much flame for brisket.

@Bishop84 doesn't the high pressure fuel line have a ball shaped connection at the pump with a big 17 or 19 mm nut to tighten it down? I've seen those get left loose all the time

I checked all connections close to the high pressure fuel pump and could not visually see any fuel coming from them. I actually couldn't see the issue from the top side of the engine at all. What I could see from the bottom, I wasn't even sure if that was the correct area it was coming from, but it was simply the highest point where I could see the fuel.

I'd love to attach the video, but this site doesn't allow MP4 attachments.

Update: spoke with the dealership today. The official explanation for the fuel leak was that an O-ring went bad in the injector system and it was not related to the previous repairs. They replaced all of the o-rings under warranty as a precaution. I suppose this would explain why the leak sealed up after a few seconds. As if the fuel caused the O-ring to swell up again.

As for the squeal noise: they still can't get the truck to do it long enough for them to be able to tell what it is but they do hear it.

As of this post, the dealership still has the truck while they're waiting on payment from the warranty company. I believe they're going to make another attempt at replicating the squeal noise.

sounds like they have not pinpointed the source of the leak and are covering their ass

Ask them if they checked the hardline fitting at the high pressure pump

I believe he initially thought that was the issue when we spoke as I dropped it off. However, when I had watched those lines when recreating the issue, there was no leakage. Additionally I attempted to see if any of the fittings were loose and didn't find any that were.

I'm not too worried about it. It gives them a chance to try and replicate my other issue again and, in the meantime, they're also covering the RAV4 rental I have that has just over 1k miles. I'm not complaining about 33 MPG at 80 MPH when my Taco MAY get 24 at 70 MPH on a good day.

I'll know soon enough. But how do you figure? An O-ring seemed like a perfectly logical explanation given where I saw the leak coming from (or the area anyway).

Finally got around to getting the video uploaded so I could share it here. I didn't keep the camera on it very long as I didn't want to be in the path of the fuel and there was really no good angle to view it. This was taken from the driver's side underneath the truck on a cold start. You can see the exhaust and O2 sensor in the foreground for reference.
