How do I pop this out to put in an OEM fog light assembly? Do I need a completely new trim piece that accommodates a fog light assembly or will that insert just pop out?

2nd gen access cab

you shouldn't need a full new trim piece, you'll have to unbolt the inside fenders and the filler piece will unscrew from there
