I'm going to a dealer to look at a used 2015 double cab Off-road 4x4 with a manual transmission. The truck is a one-owner trade-in, and it has about 12k miles.

Is there anything specific I should be looking for when I go see it?

I would make sure it had never been in an accident.

Carfax and AutoCheck are both clean. I am going to have them put it up on the lift so I can inspect the undercarriage for rust and/or other damage.

My old Tacoma was smashed to pieces, and in the body shop for 48 days and always showed a clean Carfax, don't trust those reports alone

Other than oblivious frame/body damage and mis-aligned body seam lines, what else should I look for to make sure it's accident free?

Get the dealer to put the truck on a hoist so you can personally inspect the underside of it. Being low mileage there shouldn't be too many overly worn parts since before resale the dealer would Normally take care of such things.

But just from my past experience let's say... someone I know owned a 2nd gen, high centered on it a few times, pin striped the hell out of the paint to the point it had gouges in some places that wouldn't buff out.... So this friend sanded the whole truck down and repainted it himself... It was a lease.... Just make sure some jackass owner didn't beat the truck up too bad before returning it.

A really good indicator would be mud/sand inside the frame rails since they're still box-beam construction

I'd look for an Automatic. The MT sucks. I bought a 2013 OR MT and traded at 10,000 miles for an Auto 2014.

    Why does it suck?

I've driven manual vehicles my whole life up until my current vehicle, and I absolutely hate the automatic.

First is geared too low for starting forward motion. 5th and 6th are geared too close together and 6th is geared too low for an overdrive. You'll be at 2500rpm going 70 versus 2100rpm in the automatic.

not to mention throw out bearing issues and shitty shifting..... not much good has been said about the 6 speed manual in the 2nd gens... only time you hear the good is from the manual fanboys.

Stick with what you know. Some people prefer the auto, others prefer the manual. I've been driving manual for 10 years, bought one auto and hated driving it everyday after the first month or so.

To each his own... But to say manuals suck is an extremely bias statement.

I've got my 6speed 3rd gen and couldn't be happier, especially considering everyone that bought an auto has been having some kind of problem. I havent had any issues with my truck aside from a squeeky window wiper blade and a slight clunking everytime I shift which has now been fixed.

In lieu of the above comments, I will say this is coming from experience driving a 2010 access cab 5 speed manual. Didn't go with the dbl cab cause I felt the 6 speed was too new to have all the kinks worked out. @nd4spdbh & @EdinCincinnati make good points. Be careful of that. Go with the 3rd gen, or wait for the TRD pro. "Good things come to those that wait"

The MT in the 3rd Gen was reworked to fix the complaints about the 2nd Gen MT.

If I buy a 3rd Gen, I'd very seriously consider the MT, but the 2nd Gen is a disappointment. And I prefer a MT.

I will say that the 2nd Gen MT is geared very well for towing a trailer though.

I'd check for rust. Premature rust shows up whenever a vehicle has been used on gravel roads, and sanded/salty roads. Before I bought my 2015 new, I tried a 2015 from New Brunswick that had been a rental, and it was solid rust underneath - no paint whatsoever! I live in a snow belt, but I keep the truck pressure washed, and so after it's first winter it still looks good underneath.

Other than potential rust and accidents, it's a new truck. There could be a hundred reasons why it's there. It's nice to let someone else pay for initial depreciation. Good luck!

Lol dont listen to this guy. I have a 13 OR MT and i love it. I no longer have to listen to my auto trans hunt for gears on the highway. And it has an overall better feel.

I had an 08 6 speed and now a 2015 6 speed, no issues, no complaints.

Same here. Been driving manual my whole life as well and love my 13. I'll agree that you wind the engine up a bit in 6th gear on the highway, but if you're pulling anything or doing truck stuff the gearing is great. I have a 5 speed Honda accord and had a 6 speed dodge 2500, the Tacoma trans is somewhere in between on the smoothness scale. Not as quick and tight as a small car obviously, but doesn't have the rough feel of a big truck trans either. Enjoy!
