I have an 06 Tacoma V6 4WD with 250 thousand miles on it. Have not had any engine problems in the past. Drove my truck home last night pulling the boat from the river with no problems. Went to start it this morning and it had a really rough shaky low idle and was white smoking from exhaust for about 10 seconds, then if just flattened back out on its own and ran fine for rest of day. Checked it with my diagnostic tool this evening and I attached a picture of the codes it threw about missing cylinders. Any idea on what it could be? Thank you in advance!

Check your oil for coolant and coolant for oil.

pull plugs ..........label & keep in order

do a cold compression test.............write down numbers

    test more indicative than colds

Do both a compression test static and dynamic to ensure ring and seal integrity both on cranking and running. Look for no more than 20 psi variance between each cylinder on static and no more than 5 psi variance on dynamic.

Follow that up with a cylinder leakage test when cold. Your head gasket is likely failing when cold and once the engine reaches a particular temperature the expansion allows the gasket to reseal, restoring good running condition and proper drivability.

Start there and let us know. Tag us/ quote us so we can keep tabs on what's happening.

Sounds like a headgasket. It runs poorly for 10 seconds when started because it's choking on the liquid coolant that seeps into the back 2 cylinders overnight.

A chemical block test will tell you more than a compression test or looking at spark plugs. Also watch coolant level to see if its slowly going down over time

if you can smell coolant on start up, don't waste time, doing compression tests, or even compression gas tests in coolant.

my head gasket has been going for months and months to be honest more likely a year. I could hear the gurgle in the heater core (it would come and go), bubbling in over flow,(come and go) slight coolant loss,(come and go) had occasional miss fire on start up, which lasted a week, and stopped for past 6months, and a month or so ago, I could smell coolant on start up, it as never over heated. So, 2 days ago I finally tore it down and sent out the heads for machining, and likely Ill be putting back together by this weekend, I own a garage, and as they say a mechanics car is the last one to get fixed, as it always gets bump out of que for a customers car

I see you have a code for incorrect purge flow, if your purge valve is stuck open and your fuel tank is full or near full it could pull raw fuel fuel from the tank and cause an extremely rich condition which could also cause white smoke and rough running. While it may not be your issue it's worth checking into before assuming headgasket.

A loose or poorly sealed gas cap will also throw those EVAP codes. Is the smoke just white or bluish white? Maybe it's just startup condensation. Tighten the gas cap, clear the codes and report back. Compression test would be next if the codes return.
