So my blind spot monitoring does not work nor does the parking assist work. The automatic cruise control does work.

Where are the exact location of the sensors on the front and rear bumpers?

Relentless Armored! Too many others to list.
They're not standard on all trim levels. The parking sensors are the 2 black sensors on each side panel of the rear bumper, the BSM sensors are hidden behind those same panels near the corner of the bumper.
The front does not have parking sensors on 3rd gen's, and the TSS module is located in the grille for the adaptive cruise and collision warning functions

If you get under the truck and look at the backside of the bumper, you might be able to see the BSM sensors. At most, you'll be able to confirm they're plugged in. Also, make sure no mud or debris is covering the plastic bumper caps, as the sensor can't see through obstructions.

Most likely, a dealer visit will be recommended.

Did you happen to go through deep water when BSM stopped working? If so, try spraying contact cleaner on the BSM connectors, let it dry out and then drive around (maybe about 20 miles) with the message on the cluster saying BSM Not Available. That’s what I did and my BSM cleared itself up and has been working ever since.

    It is worth a try.

Are the buttons on the dash turned on? Both systems can be turned off. I work with a guy whose son backed into another vehicle because 'the system didn't tell him he was going to hit something'. Except the dash buttons were turned off....

As others had said make sure the sensors are clean I had to replace mine under warranty after my 2nd winter in the truck the frozen snow and ice killed the sensors in my bumper.

Just an FYI: Adaptive cruise, parking sensors, and BSM sensors are all unrelated and different. All Toyota's have adaptive cruise now. It's a standard feature of the Toyota Safety Sense system, not part of any package, so having it doesn't mean you have either of the other two systems.

I took my through a car wash that had the undercoat sprayer. Blew it dry with the air dryer, and they have worked ever since.
