I worked for a large corporation that is traded on the stock exchange and Dana Corp would be a supplier/Tier 1 supplier. If Toyota engineering and quality was run the same way as ours, and I'll assume it is as it's automotive, Dana Corp would make the frame to drawing and material specs supplied by Toyota. Dana Corp would have some input but, Toyota has the final word. The frame assembly would go through Toyotas' Production Part Approval Process (PPAP).
I have seen engineering not sign off on components because the supplier sent in an incorrect steel material certification or the component was made from incorrect steel. In this case, the component material spec was to be cold-rolled material and it was made from hot-rolled pickled and oiled. Cold-rolled has a greater tensile strength than hot-rolled P&0. Would it have made any difference to the components function, no, because the components had no structural support in the way it was used but, engineering said no, the supplier has to use the material spec'd out on the drawing. I've also seen where engineering has refused to correct an issue until the consumer demand became an issue, quality control and production forced the issue, and quality cost was so high it affected the bottom line.
I know of a person that had a Tundra and had his regular oil changes done at the dealer, he liked the free wash. He brought it in one time for it's timed oil change and the dealer wouldn't let him have it back, hole in frame from rust. I believe at that time they gave him 150% of the value of the truck. So how long has Toyota let this frame issue go on?
Toyota is gambling on how alert to notices sent to the consumer, if the consumer reads it, and consumer observation of rust on frames.

From what I can tell from the picture, it don't look normal to me. It appears to be rusted from the inside out. From my experience taking pictures to prove quality points, try to get a light inside the box section or behind the frame, then take the photo. It may show up better.

Definitely a hole. May not be 10mm to qualify for replacement. Being an 09 time is running out. Don't think its worth the gamble.
