Don't be silly. 4-Lo only goes up to 35mph in a roundabout. You'll do a barrel roll otherwise.

Winter, almost daily (snowy gravel uphill driveway).

Other seasons, at least once a month for maintenance.

I put it in 4WD almost daily in the winter if there's snow on the ground. Not on the highway unless it's snowing hard and going <60 MPH. On average 95% of the time it's in 2WD.

Just curious, I usually have no trouble switching from 2WD to 4WD, but going the other way sometimes the knob won't turn at first (mine's a manual)--seems locked. Usually when I push in the clutch it will turn. Speed could be anywhere from 5-40 mph. Is there some trick to this that I don't know about? (Yes I could read the manual, but what's the fun of that?)

You should put your skewp in 4lo once a month , or it may stop working.

You only have to push the clutch when going in or out of 4LO. Not needed for 4HI, sometimes it takes a second or two to engage or disengage.

Depends on the time of year; off-roading in the warmer months, and when the roads get bad in the winter. So, on average once a month. Enough to keep everything all greased up.
