I have an 03’ 4x4 5spd Tacoma. The truck has what sounds identical to a tire hum while my foot is on the gas. As soon as I let off the gas the sound instantly goes away. Sounds kind of like it’s coming from the front of the truck. I don’t even have to be accelerating, I could be doing 50 left off the gas and he noise goes away. As soon as I touch the pedal again the noise is back. Tires are brand new, it is not the tires. Any ideas?

Possibly the fan clutch has failed and has the cooling fan permanently on, causing the noise you are hearing.

To mimic that type of sound I'd guess possible exhaust leak or ignition/timing type issue.

This could be possible. How would I check this? When should the fan be on/off?

I’ve made sure the exhaust is leak free. Not sure about the ignition/timing. Truck runs fine otherwise

Rereading your post after seeing what xtrmwhlr wrote, fan clutch stuck engaged is plausible. That's usually described more as a roar not so much a hum. But maybe there is something going on with clutch or pulley bearing. Worth checking out though
