Just got an 03 Tacoma with 76k on it, 4 cyl, 4x4, auto. I have been having this rough idle problem too. I checked the motor mounts, changed the spark plugs, used Z-Maxx, changed the oil, put K&N filters in (both oil and air). None of this had much effect, but it is a little smoohter. I was planning on running Seafoam through the vacuum line and gas tank tomorrow, but after reading this I'm taking off that heat shieldand takin a look at that manifold! I hope that is the problem because it is still under warranty and I'm gonna go ape-shit on the stealership if they don't fix the problem. Love the truck, but already had to bring it back there twice because of stupid mistakes on their part. Very helpful thread.

Well, I found out this morning that the person who had it before me already had the exhaust manifold problem. Took off the heat shield, and holy crap that's headers on there. Explains why the truck is a lil louder than the average lol. So, there is no leak there, so I am completely stumped on what the rough idle could be. Still have to do Seafoam, bought it today, but got called into work before I could do it.
