Irritating Head Scratcher

The driver's seat belt in my truck keeps getting twisted. It's irritating because I have to remove the bottom bolt, flip it 180 deg & bolt it back. I have NO IDEA HOW THIS HAPPENS. It's bolted flat, top & bottom from what I can see. Is there a pivot under the cover at the top? Has anyone had the same issue?

 

Yeah mine kept getting twisted up, so my solution was not to wear it anymore.

 

The secret is to not be careless when removing the seat belt and not allow it to twist in the first place.
