We hadn't this problem with my moms cat, Bronson. He's about 8 years old now. But he wasn't able to keep food down. Barfing and pooping outside the box. Most of the time it was because he just couldn't make it there in time. He would try occasionally though. So sad to see. They ran tons of tests and nothing was really coming back. He lost a few pounds and we were at wits end. Not knowing what to do. My mom changed up his diet tons of times. Now he gets boiled chicken and mashed potatoes morning day and night. Since then he can digest food and keep it in (both ends) he's gained weight back and is much much happier. We kinda figured dry cat food was too harsh on his intestines and wet brand food just wouldn't stay down.
Maybe try changing the diet, best of luck. Hope she gets better!

Abigail still has had a few accidents (#2) over the past week or two outside the litter box (few times it was like half in and half out) and a few times on the rug in the living room. She doesn't go anywhere else except in the litter boxes, half in and out of the litter boxes or on the rug in the living room which is weird (but good!). I still think she's just a weird cat that her litter boxes aren't "clean enough". She has 3 litter boxes in the same room that my wife or I scoop multiple times per day and then every 3-4 weeks we completely empty, scrub and put new litter back in.

As for eating... She is doing much better in that aspect. She won't eat any dry food (as you mentioned above it might be too hard for her). She gets wet food in the morning (after we give her medicine for hyperthyroidism) and then she gets more wet food in the evening around dinner time. Sometimes I give her a few other helpings of wet food if I'm home at random times. Just trying to help her put her weight back on. She was down to 6lbs I think the doctor said. Back in the day she was 12lbs.

She still pees outside the litter box at times. Fortunately we have pee-pads so it's an easy cleanup. Sometimes it looks like she's "spraying" but other times it's just outside the box down low (on the pee-pad).

Few pictures I have of her on my Imgur from a few years back. She does make me smile even though I have to clean up piss (daily) and shit outside the box/on rug (few times a week).

Glad to hear she's doing better, it'll take a while to put the weight back on but I'm sure she'll be just fine! Yes as fur parents we have to clean up piss n poop in the weirdest places, but we love them anyway hahah. She's adorable by the way. Best of luck!
