    A patient came in who had jumped off a high balcony to save himself from being shot. He was not shot, but as you will see in the xray, he will not be walking any time soon.

    For people who don't know, you have two bones going from your knee to your ankle (tibia and fibula). This xray shows the distal parts of these bones to be in many pieces, and his right foot to be displaced 90 degrees....
    It was not a open fracture (broken through the skin) but it sure was enlarged and quite bumpy.

    to tell you the truth, I am not sure.
    extensive surgery to pin the shattered tibias and broken fibulas. I would say no rehab, but probably won't be able to run the same because loss of range of motion a bit. But I would guess against rehab, just long LONG healing time.

    that will require several surgeries, rods, plates, screws and bone fusions, if that stuff works, which is no guarantee, he will always have pain, probably wont be able to work on his feet ever. i would think it would be a year before he was up walking around without crutches or a walker and thats a stretch

    my hospital would shit a BRICK if anyone were to take and post pictures like this. in fact, a girl got fired recently for posting a pic of a patient on facebook. whether you can identify the patient or not, pictures like this are absolutely forbidden.

    x2. My facility would shit a brick. I have my own little file of xrays of bad bad fx's, thing in orifices that should not be there, and other weird stuff. Even though I cut off ALL identifying information....If I was caught making copies of these xrays and leaving the facility with them, I would more than likely be terminated.

    BTW...that bilat tib/fib fx is nothing compared to some of the stuff I have seen. Level I trauma center = seeing some of the gnarliest shit ever!

    I don't know HIPAA inside and out (if you're in healthcare, even for as short a time as I have been, you hear enough about it that you tune it out), but whether or not this is legal, you can't think of it that way. Don't look at it from the standpoint of "legality," instead, look at the "morality" of the issue. Suppose that patient's family could stumble upon those pictures somehow. What if it were you on that table? Would you want someone posting pictures of you across the internet?

    It depends.

    The pt.'s status prior to injury and functional goals are the biggest determinant of recovery time. This is because different activities require different levels of physiological recovery by the patient.

    For example, an 75 year old little old lady that wants to return to grocery shopping and getting her mail could recover from a bone fracture to her desired functional status in a few months.

    An athlete that wants to return to sports may have 6 months or more of rehab.

    If we assume that the individual with the bilateral ankle fractures posted above is a middle aged male with average health, he's looking at multiple surgeries and at least 2 to 3 months of bone healing before the MD turns him loose for weight bearing to tolerance, which is when real rehab begins.

    After that, it's going to take a couple months to get him back to normal strength and mobility, and another couple months if he wants to return to intense activities (say, sports). These are all minimums...most patients require more time (especially with the rampant obesity in the US today)

    He will have some range of motion loss, will almost certainly have some level of pain and swelling in the ankles for life, and may develop dysfunction of the knees, hips, and lower back due to altered gait.

    Best guess: Total recovery time 6 months (at best) to multiple years (if there are many complications, poor prior health, poor adherence to treatment, etc.)

    There are too many factors to give a straight answer. Every patient is different, and every rehab plan of care has to be a custom job based on context.
