My truck shakes when changing gears

Update on my truck/trans. I'm now at 185,000 miles. The shudder was starting to come back. 1st experience with it was around 125,000 miles. Had Toyota do the flush/service this time and its back to "like new" I told them to make sure they added in the friction additive and the service rep said they changed the WS fluid a year or so ago to include the friction additive. Since I knew what was up this time, I had it serviced at the first hint of the problem. My first miles on this truck was tons of interstate mileage. From here on out, will just plan to flush/service every 65,000 even thought the recommended service schedule in the owners manual does not call for it.
