its acting like its not getting enough gas but only in 1st and 2nd gear...almost sputtering.

Otherwise its still running like a champ

Fuel filter came to mind first but the filter is on the engine block (not under the drivers seat like some others) and I dont see me being able to change that.....if its even the problem

    Transmission?

You're in the wrong section however the fuel filter on the 2rz 3rz is actually kinda easy to get to, if i remember you just need to pull the front driver tire off and pull back the mud guards in the way first and you can access it.

Does it idle fine and just bog in 1st and 2nd gear?

If you run a K&N Filter or anything with oil check the MAF sensor as if its dirty or wet it can make the engine bog under load.

Try doing a full in engine mild tune up:

Spark plugs/wires if needed. plugs every 30-60k miles wires 120k miles or needed.

Maf Sensor and throttle body cleaning. Good to do every 10k miles anyways.

New stock OEM Paper air filter from Toyotas dealer or Napa Gold. Good to change every 15-30k or so miles depending on conditions.

The fuel filter should last the life of the truck however its now 16 years old and if never done its a $16 part at the dealer or so and a 20 minute job.
