So today when I was out in the parking lot at my college, I noticed that my leaf springs looked flat, and were not the typical curved shape that they should be. I was wondering where the best place to go to get this fixed would be, and what type of leaf to actually buy. Also how much will something like this cost?

Call a local spring shop and see what they would charge to build new leaf packs.

Is the rear noticeably lower than the front?

Jack up the whole rear. If both wheels drop then your leafs are good. The side that doesn't drop is the where the bad leaf is. If the wheels don't drop evenly replace your leafs. You can get a nice set of dorman leafs for around 110 each. Don't re arch existing leafs just start fresh with a new set.

Are you looking for stock height leafs, or lift?

Not sure I follow this, when I replaced my leaves, the axle dropped evenly and I had a missing chunk on the driver side and a cracked leaf on the passenger side.
    Truck sat level too.

Ok so this is what it looks like. It is bending where the leaf is attached the rear axle. I'm not an expert with this sort of thing, but I know that is not the way it should look. Sorry about the glare on the first picture I was in rush to snap a pic before I had class
