    As far as "cold" starting, I'm pretty sure the owner's manual says you can drive it immediately after it starts.

    Yes, get in, crank the key, once the motor catches drop the clutch and feed it some gas, and away you go. Fasten your seatbelt on the first relatively straight stretch with no braking, otherwise the seat belt brake won't let you pull it out far enough.

    (No, I don't know what the comparative procedure is for an auto.)

    Normal. If the truck has been idle with engine off, the shift to D/R will have a slight jump; visa versa when driving, backing up, etc...pretty darn smooth, but a great solid shift. This is a heavy duty trans in the Tacoma.....no problem.
