    This is probably redundant but....

New means new. It does not mean it is any good. Defective parts slip thru.

The truck will still run with a gooped up PCV, but it will run a lot better with a new one. For a Ten-dollar part at Toyota, It's worth it in my humble opinion.

I wanna say my local dealer only wanted about $3.50, it was insignificantly cheap is what I remember. About the same price for a can of brake cleaner. I think I spent more for the new hose & hose clamps.

If you need to replace the hose like I did, the Toyota one is nicely formed and it’s a hose in a hose, so abrasion protected. I didn’t feel like dealing with the stupid parking lot at my dealership for a hose. So when I needed to stop at a parts store, I picked up 2’ of 3/8” hose and a pair of hose clamps. 1’ might have been enough, I didn’t measure but I’d rather have too much than not enough. I wrapped it with a zip tie where it wants to rub.

I guess maybe if you're in a time crunch and a new valve isn't readily available, but if you're going through the effort to remove it you might as well just replace it with a new one given it being such a cheap part.

It's a 5k mile interval per the owners manual on my 4.0. I wouldn't double it because you run synthetic unless you're getting an oil analysis.
