Do you have to tell the guy who does the tune what you want done or does he just go hooks up and performs some kind of a scan to determine what needs to be tuned.

On my side, there is typically a discussion about options, fuel type, and goals.

yes maybe, if you want AGM battery support, if you want premium or regular octane (some are octane learning), some tuners offer various throttle sensitivities... just ask them what they offer and they will tell you. depends on the tune and how they do their thing.

I watched a video of this guy who took his truck in to have a OV tech go over his truck. The owner was complaining about his taco not having much power that it should have , not shifting right, gas mileage etc etc. The tech hooked up his OV laptop and went over the truck and did some tweaking and told the Taco owner that it would take about 300 miles before the truck would be fully calibrated to the new settings.
Make it short the owner left the Tech and made a video of the after the calibration and was amazed at how the truck seemed to have more power, shifting better, a few more mpg and was impressed and said he can't imagine what it will be like at 300 miles. Maybe he will bring us update later in a video.

Most of the settling happens in the first 10mi to 20mi. Driving then cycling the ignition helps also. The changes after that initial period are more subtle improvements that someone might notice doing the same, familiar drive. Like a commute.

That is way I have my flash folks go for a test drive before we're done. I encourage them to take their time and feel out the tune.
