My brother was on the way to work this morning when his 4runner decided to break / fall apart in the middle of the road after crossing a water runoff in the middle of the road. Over the past week or so, he's been complaining that his steering hasn't felt quite right. I guess it was time for something to break. Currently, it's at the dealer.

Any ideas as to what happened?

I think his ball joints were pretty new. Last year he also had installed a steering rack and lower control arms.

Maybe they didnt tighten everything to spec when the new ball joints were installed?

Definitely a ball joint, which in turn tore the inner CV joint apart.

Yep, lower ball joint. The tie rod end looks fine and the CV boot wouldn't cause it. Looks like the ball joint just gave out or the nut came off somehow and it jumped off.

Without a doubt. The steering not feeling right was a sign.
