The hole in my Gasket was half the thickness of the plastic ring on the cap of a soda bottle. It was a Very Small Hole. The Mechanic said I probably could have gone a while before it got worse. But I got it in before more damage was done. I would notice when I pulled up to e Stop Light if I had the Heat on when I let off the Gas Pedal the hot air would get cold until I was back on the Gas Pedal.

could very well be at this point, were all just speculating, needs to be diagnosed....
im guessing / assuming he keeps adding coolant to the radiator....
my guess is if leak is that bad, maybe he will get lucky & see some intermix milkshake evidence on the inside of the oil cap...
if there truly is an issue, which I dont wish upon anyone....

This is pretty much on par with the symptoms that were associated with my blown HG. Originally, normal daily driving around town indicated no issues. But, coolant pushing into the coolant over-flow bottle to the point of actually dripping out of the bottle with out over heating on weekend trips started to happen more frequently. Especially when going up a long grade like the Cajon Pass or sections of the 395. Temps were first monitored via Blue Tooth Dongle and an APP, then eventually with an Aftermarket Coolant Temperature Gauge that I installed. Eventually I started to get a Misfire condition at first start up after a long cold soak that would last just a few seconds. That's pretty much indicative of coolant intrusion into the combustion chamber(s).

See my post from a few years ago of when I installed my HG

To the OP, whatever you do, dont add any of these head gasket fix in a can quick fixes....
You won't be doing yourself any favors....
Assuming the truck is worth fixing & doesnt have a million & two miles on it...

Ive fixed many with that stuff, but the owners were just buying time as they had no intentions of repairing.
case by case, these trucks weren't pushing coolant, its was just misfires cold, coolant intrusion.
Im not saying some of them dont work, but if youre going to repair, you also just compromised the efficiency / effectiveness of your radiator...
