Would I have to regear if I go with 33" tires and 20" rims

Have to is very subjective. You will be adding a fair amount of weight with that wheel and tire combo. I think the comon line of thinking is your ok up to 33's but maybe someone with 20 in wheels can comment.

33s are 33s and it's borderline depending on your driving habits, terrain, towing etc. My guess is 20" wheels and 33s might actually weigh less than 17" 33s. But is is more than just the rolling inertia. I have 33s no rehearing and it does not hunt gears but I'm in a flat part of NC.

I have stock gearing and 285/75/16s (33ish) and don't have any issues that would make me want or need to regear.

You will not have to. You may or may not need to. All depends on how and where you drive and what your expectations are. If the truck has more than enough power for you now, it may be fine. If your truck lacks power or is borderline now, you probably won't like it's behavior without regearing.
