I'm out of ideas, and with out the truck in front of me, there's not much else I can do. I hope you get it figured out, and it's something cheap/easy to fix.

Here is the result $882.00 later. The water pump seized and spilled some antifreeze into the timing chain area. The idler came apart and caused the timing chain to jump a tooth. Took the Toyota dealership a day to figure it out after they called me a few times and asked about all the stuff you guys had me try. This was a funky one, I have heard of timing chains breaking but not jumping a tooth like that. I never saw a leak on the truck and never overheated so did not even think about the water pump.

    Thanks for all the help.

Your truck has a timing belt, not a chain, and they do jump teeth on occasion. This right here is why I said I hate to diagnose over the internet. If I could have heard it, and seen it, it would have been a lot easier to fix.

I'm glad you got it squared away, but sorry you had to spend all that money.
