The other day after leaving the house sitting at a stop light i felt a real rough idle whole vehicle was shaking, then at the next light my tachometer start to go up and down and my truck almost stalled out. I changed the fuel filter with no remedy. My truck in a 2001 4WD Toyota tacoma 4cyl 2.7 L.

    Any suggestions?

Throttle body and plugs are good places to to look, also was the fuel filter full of gas or did it have some water in the bottom? Water in the gas tank could just come back to haunt if it is in quantity. May have to drain off some fuel or try a water removing additive, but get a good one the cheap ones are mostly isopropyl alchohol. Not as good as say Techron from Chevron.
