Not sure the specifics of the pain killers question but generally we don't like prescribing them for legal reasons, I.e. pain med seekers and the whole narco lawsuits. Generally we like the pain to be controlled and then tapper down till they're off of them. It's worse if you need to stick them with iv abx therapy for a long duration if they they have a pmh of drug abuse. Mostly we just tell the patient to consult with a pain specialist as outpatient.

 

Studies are being and have been done on the use of Tylenol and NSAIDs for post operative pain. Europe has been doing it for a while as well. When I was at a field Hospital in Afghanistan, the Germans never prescribed any narcotics after surgery.

There was a trauma conference I was at and a doc from University of Kentucky was giving a dissertation on how they have done controlled studies in post op pain management and the "non opiod" group had better recoveries and pain management by landslides.
