I would fab the ramp with some type of screw mechanism to raise/lower and fine tune a bit. Maybe have them set back off the end so you have some ramp laying on the semi truck and flat bed.

 

Another good idea is to put a wheel on one of the long sides parallel with the ramp plane so you can roll it instead of carry it. We have one at work to load and unload tool boxes from Sea lands in the reactor building. The wheel makes the job much easier.

 

New Plan! Ima make the truck driver back in to the parking lot by me blocking traffic. And Im going to rent a forklift with extensions. the flat bed route is getting expensive

 

5000lb forklift is $165 a day + $45 for the longer forks, VS flatbed and time. Just hope this driver is good lol

That sounds like a better plan. That puts the forklift operator liable if something gets fucked up between the trailer and the ground. Last thing you need is to drop that table
