I have them in 0 offset 4.5 bs and they might stick 1/4 inch? so with the -12 I would think about 3/4 inch?

Changing out wheels/tires should not require recon inspection.

If your tires stick out beyond the fenders, that will be a PMVI/Safety inspection failure.

And the chances of you getting pulled over for wheels sticking out past fender flares will be rare. I’ve never heard of it actually happening in recent years, unless it was during a truck meet where people drew too much negative attention by doing burn outs and road blocks.

Now, if you happen to get pulled over for something else, like speeding, then HPD could add that infraction.

HPD might even go as far as to contact your inspection station to see if you altered your truck specs or if they passed you with illegal specs. If the latter, HPD/state government could revoke that inspection station’s inspection license.

I here ya, the pics I gathered and looked at seemed like only the sidewalls passed the outmost fender, that being said and correct me if I'm wrong but isn't it illegal in hawaii if the tread not sidewall cannot extend past the outmost frnders?

There are many thing illegal in every State, but doesn’t mean the law enforcement will actually enforce that statute.
But if you’re asking if you will pass recon inspection if the sidewalk sticks out beyond the fender flares, it’s at the discretion of the inspector if it’s questionable.

I think you’ll be fine. How many trucks do you see with almost the whole mud tire sticking out and they never get stopped

I think you'll be fine. They should fit nicely inside the fender well with the flares, especially with the 265 tire. The OEM wheels are 4.5 BS, so it isn't much of a difference.
