What brand are they?

 

They are ez-accessory brand, list price is around $80. These are new, I never used them. I just used a 5mm spacer plate as my wheels already had offset, for me the 5mm spacer worked to clear my calipers.

Are they hubcentric spacers?

 

They are lugcentric. Just as are Toyota wheels that use conical lugnuts.
