    Safety has a lot to do with circumstance. Gemini didn't fail alot because the tech was simple and really well tested out on unmanned launches. Apollo was low failure because after the deaths during Apollo 1 and the failures of the early Saturn rockets an internal microscope was placed on safety. That and Apollo 13 might be the luckiest accident of all time in that the design of the lunar lander provided basically two usable spacecraft with completely redundant systems. The shuttle was, well, really not that safe to be honest. Sure it had a high safety record but any issues that disabled a critical system during launch or return were immediately fatal due to the lack of any true usable ejection or removal system. All of the shuttle accidents were traced back to a lax attitude amoung NASA employees, and not the shuttle itself, which I believe is still probably true. Time will tell if private industry cares more. I'm not sold that Elon will care about failures and deaths beyond the insurance costs.

    Marginalize the individual sure, but hating paying for premiums, legal fees, etc (and stock price hits on failure) do provide a lot of leverage to get it right.
