Did it the 2nd time today since I've owned the truck since new.

Never a good sound or feeling when it makes that harsh metallic scrape.

I know these trucks obviously don't have anything built in to prevent it from happening (like some luxury cars have such a feature), but was the engine supposed to cut out when it happened? (had to cycle the key to off and then crank it back on)

I feel like the first time I did it, the engine kept running.

I could see it stalling the engine if the starter gear hits the fly wheel just wrong.

Your not the first or last who has done this. Just try not to do it again and honestly your truck is probably gonna be just fine.

You'll ruin the starter before you hurt the engine.

I've also done this once on my Tacoma. When I changed the clutch, I removed the flywheel and starter and the gears on both looked like new, no visible damage at all.
