Or insurance covers the MINIMUM standard of care...Like an amalgam filling....at full reimbursement...However there are no dentists who still use them as they are considered hazardous/unsightly. The standard of care in first world nations is a color matched cement filling...BUT THAT'S only covered 40% or something...whatever the cost of the Amalgam is just deducted from the cost of the cement...

OH, and if you chip off a chunk and want a 3d printed on lay you're getting nothing and like it (but still worth it)
