Has this happened to anyone else? It doesn't seem broken. It just falls out sometimes when I fold the visor down.

Take the bracket off the headliner and I'm betting you'll find the internal retainer broken.

Side note, coordinating nail polish with interior color is a classy touch!

I’m with Bill. Busted. Maybe if you take the bracket off you can get a diy fix going?
Replacements aren’t too bad but they aren’t cheap either.
This post has some part numbers, there’ s several of different versions based on options

This happened to the driver side visor in my wife's Camry. There was no fixing it, she needed a new one. If I recall, it was about $90 for a new one, but who knows for the Taco.

Same thing happened to my truck the week I bought new in 2018. Easy fix. Just need to unscrew the bracket and reseat the sun visor rod back into the retaining clips in the bracket. Hasn't happened to mine since.
