I changed the spark plugs out with .40 gap. Truck shakes after start up and the yellow check engine light is still blinking on and off. Is there something else I need to do?

Are you using OEM stock Denso plugs?

Was the check engine light on before? If so the plugs were most likely not the issue. If not, Check wire to plug connection. There is a possibility you cracked the ceramic insulation of the plug on installation or the plug was bad to start. You may be able to get a code reader to tell you what cylinder is missing or unplug one spark plug at a time and see which cylinder makes the least difference unplugged

Was the CEL flashing before the plug change?

Until then, make sure all the coils are plugged in good.
Those locking clips like to break off after years and years of heat cycles.
If yours broke, use zipties to hold them together.
Just "pushing them in" won't hold them in good enough.

Did you use copper plugs or iridium? On iridiums, I’ve seen damage occur when checking/setting gap to either the tip of electrode or the little “disc” on the ground electrode gets broken off.

Denso correct gap is .7 to .8mm. That is like .03”. For 3rd gen anyway

Gap specified in the 2nd gen user manual should be 0.044”.

I'm guessing one or more of the coil connectors are disconnected or not fully seated
