Toyota has been looked at, fined, recalled for unintended acceleration. it's probably the same or similar software algorithm in all type vehicles with obvious variations. But even NASA couldn't find anything wrong or duplicate the issue.
I find it hard to believe that if you put it in neutral, slam on brakes or turn the key to accessory as not to lock the steering wheel it will not stop, but the drivers swear otherwise.
    We may never know

I thought that brakes were always designed to overpower the engine regardless. For this very reason.

I said earlier in the thread that I've had the truck surge through the brakes. On a couple of occasions I had to press them much harder because the surge when you put it in drive is so bad. Same with reversing.

I haven't noticed it too much recently though. Either I got used to it or the tsbs have addressed it.

The lady in the article said it was in park. I've never had it surge in park.

Sorry if I am wrong as my DD is a manual, but shouldn't you always hold the brakes as you shift from D to Park?

You have to in order to shift, but you don't have to hold them hard. In the morning when I cold start my car and the RPMs are high as it warms up, if I'm just lightly holding the brakes to shift to reverse, it will start rolling backwards.

No they are the same. The "white stripe" you mention in the top picture is the same one that the shut off switch is mounted to in the second picture, it's just cropped differently and they replaced the shut off switch/cover due to safety.

The blocks are not different, though the angle is extremely different so they look a different size, also could be due to different focal lengths for each shot. In the first shot you can see the propane tank case just at the edge that you see in the second shot from the opposite angle.

It was probably replaced - the cover was damaged and knocked off. They probably put an entirely new switch box on.
