Hate to bump this old thread but this issue seems to be what I have going on with my 1999 with around 300K miles. When I got the truck the previous owners had "fixed" the issue with a very stiff spring attached to force the throttle return spring all the way closed. This was problematic as driving a manual transmission vehicle and always having your RPMs dragged down isn't fun. If my foot wasn't on the gas the truck was rapidly shooting down in RPMs.

Attached is a photo of where the spring sat. As well as a few videos. If I'm not physically pressing the linkage/return system down all the way the truck will just idle at around 1000 or higher rpm. I would go grab a junkyard unit, but finding one for a 5VZ with a manual transmission will be almost impossible. I just did injectors, valve cover seals, throttle body cleaning, and IAC cleaning. I also bench tested the IAC before and after cleaning it. Really think it's more of a TB issue, especially after adjusting the dash pot bolt and idle stop bolt.

Did you look to see if you have a gap around the butterfly and the throttle body wall after you cleaned it?

Yeah it’s shown in the first YouTube video I linked. Gap wasn’t as visible until I cleaned things out.

The gap on the top of your butterfly is probably your issue,
Send the throttle body off to maxbore.com. For 125.00 to have it fixed.

If you have a gap and air is getting passed your problem is the butterfly and throttle body, both have wear and need to be fixed.

I’m also worried about how the throttle won’t return all the way. I can get it to idle if I force it all the way closed.

But multiple people have mentioned they’ve never seen that part of our TBs fail. So maybe the butterfly is getting stuck against the wall (there are wear marks inside the TB) and I’m just forcing things a bit more closed when I press on it.

When the throttle body is rebuilt it is completely taken apart springs and all and put in a sonic tank.
I would go this route, it’s obvious the butterfly and wall have issues regardless

Already on it's way to Maxbore. $150 is extremely reasonable after looking at his process.

Excellent, this service is a bargain and should fix your problem,

If you have the extra time I took a dremel with a wire wheel on low speed and cleaned the complete throttle body and made it look new again

If it was my 04 getting serviced I’d definitely be cleaning up the throttle body. This 99 has seen some terrible things so I’ll settle for the engine bay not being covered in mud anymore.

Can't test it out since I'm doing body work on the truck but hopefully that fixed the high idle.

My 99 is far from returning to the road. Timeline is maybe finishing it by late October. Just got the TB bored out so it wouldn’t hold me up after frame and body repairs.

Oh okay. I have my original TB that I want to get bored out. I replaced mine with a junkyard one.

I was curious if it fixed your high idle. I had a similar issue and one of my issues was the TB but another compounding, weirder issue was that I had a thermostat that was stuck open. That was keeping the engine from reaching the operating temperature for the computer to bring the idle down. Because the computer keeps the idle high until it warms up, but mine wouldn’t warm up because the coolant was circulating from the moment I started the engine. Good luck!
