    Toyota recommend changing plugs every 50k, so it would be money well spent.

    Does the rough idle happen ALL the time or just cold?

    Try using someone elses gas at the next fill up and notice if it isn't the fuel.
    We're going into winter fuel which means more ethanol in the fuel absorbing water (= rough idles).
    The water gets mixed with the ethanol because of condensation in the tank overnight.
